Connected Care at SickKids partners to create a seamless experience for children with medical complexity and/or technology dependence and their families as they transition home and across the paediatric health system.

Our Services

Paediatric Education

Our program provides competency-based education to support the unique care needs of children with medical complexity and/or technology dependence. We offer a wide range of education from interactive workshops, webinars and eLearning that are delivered in-person, virtually or hybrid. Explore upcoming sessions.

Virtual Care

Our program provides virtual care and consultation for patients, family caregivers and healthcare providers using digital platforms to provide support following discharge and guidance in paediatric assessment and management of common problems related to a child’s medical technology.

Care Coordination

Transitional care coordinators collaborate with clinical teams and family caregivers when planning for transfer to another hospital or discharge home and to the community. This may include support with funding programs, home equipment and out of province resources.
