Connected Care hosts Advisory Tables that incorporate the voices of patients, families, and hospital and home and community care partners in the design, implementation and evaluation of the program. Membership of the Engagement & Integration Advisory Table includes representation from across paediatrics and supports development, implementation and evaluation of the Connected Care Program.
