Education
Connected Care provides timely, up-to-date teaching for patients and family caregivers who are preparing to go home with new medical technologies. Teaching may also be provided to home care staff including nurses and personal support workers to expand their home support network. Education may be provided before discharge home and is available ongoing after transition to home.

Discharge Teaching
One-to-one, personalized education allows patients and family caregivers to learn and practice skills in a family- friendly space in the hospital. Classes are available seven days a week, morning to evening. Interpreter services are available.

Just in Time Training
We support patients and family caregivers with education as needed after going home from the hospital. This training is personalized to each child’s care plan when additional caregivers are needed. For example, a relative or nanny may need to support the care of your child or when a new home care or school nurse joins your care team.
Connected Care on the Go
We provide quick delivery of paediatric teaching supplies to learners to support training when they are unable to come to the hospital for an in-person class. A customized box is delivered and includes teaching supplies, teaching dolls and resources needed to practice skills being taught virtually by our team.
Home Care Provider Training
Our team provides education for healthcare providers in home and community care who are caring for children with medical complexity and/or technology dependence. This includes home care nurses, personal support workers, daycares, and schools. Education is offered both in-person and virtual on a variety of topics.
Connected Care at the Bedside
This service allows home and community care providers to come to the hospital bedside as learners to meet the patient and family. Opportunities also include reviewing the care plan, patient specific training, and hands-on practice of skills. Learners are provided with a summary of what was covered and information to continue confidence and competence in knowledge and skills.
Unregulated Care Providers
Connected Care is developing services to support training for unregulated care providers such as personal support workers, family members, nannies, and schools.
Virtual Care
Connected Care Live
A web-based platform offering real-time support for patients and family caregivers with specific types of medical technology to ask questions, review skills, and troubleshoot issues. Connected Care Live is also available to home care providers who are encouraged to partner with families when accessing support

After Discharge Visits
Our team will continue to support patients and families after discharge. Initially, we will attempt to reach you soon after discharge home by phone or video to review safety check lists and understand/fill gaps/needs to ensure successful transition home.
Ongoing Follow-up
After discharge, our team continues to be available for patients and family caregivers to support their confidence in caring for their child’s medical technology. These follow-up appointments can be in-person in our clinic at the hospital or virtually through phone or video.
