1) Not for Emergencies. CONNECTED CARE LIVE IS NOT AN EMERGENCY SERVICE. Please call 9-1-1 if you need emergency assistance, or contact your Patient’s primary family caregiver.

2) Use of Connected Care Live. SickKids grants you the right to access and use Connected Care Live solely for the purpose of providing or assisting in the provision of health care to your Patients, and only with valid login credentials. SickKids will provide, as part of Connected Care Live, access to:
(a) A real-time consultation service, available via online instant messaging, video chat and telephone, during the posted hours of operation;
(b) Online educational resources and updates self-directed educational resources and updates (which SickKids may add to, remove or modify from time to time); and
(c) A virtual community of practice to connect you with other home care providers and share knowledge.

3)You are Responsible for the Care you Provide. The information provided through Connected Care Live is intended to aid and support you. It is a complement to existing resources, protocols and policies, as applicable, that may be provided by your employer. This information is not a substitute for your own diligence, professional judgement and obligation to provide competent health care. You remain professionally responsible for the care that you provide.

4) Protection of Personal Health Information, and Consent. As a home care provider, you must uphold your obligations to protect a Patient’s Personal Health Information (“PHI”) and respect the consent they provide. That means only sharing PHI through the consultation service when a Patient has given consent, and only if that consent has not been revoked, or limited by exceptions. Information shared over Connected Care Live is stored securely with a third party, consistent with SickKids privacy and information security practices. 5) Availability of Connected Care Live. Connected Care Live may have errors or be unavailable sometimes. SickKids can, at its sole discretion, restrict or suspend your access to Connected Care Live if you breach the Terms and Conditions, or for any other reason. SickKids may also suspend or terminate Connected Care Live at its sole discretion.

6) Monitor Use. We track usage of Connected Care Live and record interactions, in order to ensure patient safety, see how users are interacting, and make improvements in the services.
