Home and Community Care
Education
Connected Care provides competency-based education for healthcare providers in the community who care for children with medical complexity and/or technology dependence. Create an account to register for any of our courses, access e-modules, and resources. We offer education on a variety of topics, delivered in different ways to meet the needs of learners.
Workshops
Workshops offer the opportunity to review theory and apply skills using simulation. Virtual and in-person workshops are available.
Webinars
Review foundational knowledge on paediatric care and specific medical technologies in a live webinar.
Just in Time Training
Just in Time Training supports a patient-focused session when home and community care providers join a new patient and family caregiver care team. For example, a new home care or school nurse is assigned to a new patient.
Connected Care on the Go
We provide quick delivery of paediatric teaching supplies to learners to support training when they are unable to come to the hospital for an in-person class. A customized box is delivered and includes teaching supplies, teaching dolls and resources needed to practice skills being taught virtually by our team.
Connected Care at the Bedside
This service allows home and community care providers to come to the hospital bedside as learners to meet the patient and family. Opportunities also include reviewing the care plan, patient specific training, and hands-on practice of skills. Learners are provided with a summary of what was covered and information to continue confidence and competence in knowledge and skills.
Unregulated Care Providers
Connected Care is developing services to support training for unregulated care providers such as personal support workers, daycares, and schools.
E-modules & Self Directed Resources
Learners can complete online education in their own time with on-demand webinars including education and practice resources to support caring for the paediatric population.
Connected Care Live
Connected Care Live is a 24/7 virtual platform led by nurses offering real-time consultative support for home and community care providers caring for children with medical complexity and/or technology dependence. Ask questions, review skill, and troubleshoot issues with a SickKids health care provider.

Education
